Job Summary

We are seeking a Registered Nurse or Licensed Practical Nurse to join our Hospice division. This is a full-time position that offers flexibility and the opportunity to make a meaningful difference in the lives of our patients.

Responsibilities
  • Conduct ongoing assessments of patients' physical and psycho-social status, and develop plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and their families.
  • Provide education and interventions to support effective management of patients' pain and other physical symptoms, and assess their response to treatment.
  • Ensure that necessary equipment, supplies, and services are available to support patient care.
  • Participate in IDG meetings and make revisions to the plan of care as patients' needs change.
  • Document patient services accurately and in a timely manner.
Requirements
  • Active, unrestricted license as a Registered Nurse or Licensed Practical Nurse in the State of Michigan.
  • At least one year of relevant experience as an RN or LPN.
  • One year of management experience (preferred).
  • One year of home health experience (preferred).
  • Current CPR Certification.
  • Reliable transportation with a valid state driver's license and current car insurance.
