As a Wound Care Nurse, your responsibilities will include coordinating thorough wound evaluations and care strategies, executing treatment plans, and assessing results throughout the facility. You will also act as an educational resource for patients, families, and healthcare staff, offering training and updates on optimal wound management, prevention, and care methodologies.

Conduct wound assessments, follow-ups, and management for all patients in accordance with established policies and procedures; develop skin care programs to prevent skin breakdown. Assist in the creation and implementation of policies and procedures for managing residents with draining wounds, fistulae, and/or tubes; wound prevention, care, ostomy care, and incontinence. Revise care plans based on the resident's response to treatment and evaluate the overall effectiveness of the plan. Assess and reassess pain levels; utilize appropriate pain management techniques; educate residents and families on pain management. Complete initial nursing assessments for newly admitted and readmitted veterans/members. Support the development of Minimum Data Set (MDS) assessments, ongoing quarterly reviews, and care plans. Serve as a resource for nursing and facility staff; provide in-services on wound prevention, care, ostomy care, and incontinence as needed. Participate in training and staff development programs for regularly employed staff. Incorporate a culture of veteran-centered care in the execution of job duties. Communicate effectively with nursing staff and physicians; consult with other healthcare professionals as necessary to address resident needs. Ensure documentation adheres to current standards and practices. Assist with direct care for Activities of Daily Living (ADLs) including bathing, dressing, eating, toileting, personal hygiene, and mobility as needed. Attend training and continuing education in wound prevention, management, and ostomies; stay updated on advancements in wound care. Actively participate in AMS processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work. Fully engage in the QAPI process, our Quality Assurance and Performance Improvement program mandated by CMS which incorporates AMS.Required Knowledge, Skills & Abilities:
Knowledge of gerontological nursing theory and practices. Understanding of residents' evolving medical, physical, and psychological care needs. Awareness of the effects of aging. Familiarity with wound, ostomy, and continence care in accordance with professional standards. Comprehensive knowledge of wound staging systems. Understanding of medications and their administration based on patient age and clinical condition. Advanced knowledge of sterile dressing techniques and wound management products. Awareness of federal, state, and agency regulations governing nursing practices and the management of medical supplies and medications.Skills:
Proficient in wound care and management. Strong technical, critical thinking, and interpersonal skills to effectively communicate and collaborate with providers, staff, residents, families, and support personnel in a dynamic environment. Excellent organizational and time management abilities. Proficient in computer applications, including Word, Excel, PowerPoint, and Google. Strong writing/documentation skills for generating weekly skin reports.Abilities:
Ability to educate and share knowledge with a multidisciplinary team involved in skin integrity and wound care. Capacity to develop and participate in quality improvement initiatives in skin integrity and wound care. Ability to interpret data regarding resident status to identify specific needs and provide necessary care. Ability to prioritize tasks with minimal supervision to independently fulfill job responsibilities. Maintain patient confidentiality at all times.Preferred Qualifications:
2+ years of experience in Skilled Nursing Facility/Long-Term Care preferred. One or more years of experience in Skilled Nursing Facility or Long-Term Care preferred.Pre-Employment Requirements:
Current Negative TB skin test or a written statement from a qualified healthcare provider indicating freedom from Tuberculosis. Ability to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety. Current or temporary license to practice as a Registered Nurse in the State of Arizona.
