Acute Transition Care Management RN

Job Summary:
Vaya Health is seeking a highly skilled and compassionate Acute Transition Care Management RN to join our team. As a key member of our care management department, you will play a vital role in ensuring that our members receive high-quality, person-centered care during their transition from inpatient to community-based settings.
Responsibilities:
- Conduct comprehensive assessments of members' physical and behavioral health needs, identifying gaps in care and developing personalized transition plans.
- Collaborate with members, care teams, and healthcare providers to ensure seamless transitions and optimal outcomes.
- Provide education and support to members and their families, empowering them to take an active role in their care.
- Develop and maintain relationships with community-based providers, ensuring access to necessary services and resources.
- Participate in interdisciplinary care team meetings, contributing to the development of comprehensive care plans and ensuring continuity of care.
- Monitor and evaluate the effectiveness of transition plans, making adjustments as needed to ensure optimal outcomes.
- Document all interactions, assessments, and care plans in a timely and accurate manner, adhering to Vaya Health's policies and procedures.
Requirements:
- Associate Degree in Nursing required; Bachelor's Degree in Nursing, Healthcare, or Human Services preferred.
- Two or more years of experience working directly with individuals with physical health, behavioral health, IDD, or TBI needs.
- Active and unencumbered RN license in North Carolina.
- Experience in Medicaid Managed Care preferred.
Physical Requirements:
This position requires close visual acuity, physical activity, and mental concentration. Ability to drive and sit for extended periods is also necessary.
Residency Requirements:
This position is required to reside in North Carolina or within 40 miles of the North Carolina border.
Salary:
Depending on qualifications and experience of candidate. This position is exempt and is not eligible for overtime compensation.
