AccentCare is seeking a skilled Clinical Manager RN to lead our Home Health team. As a key member of our organization, you will be responsible for overseeing day-to-day clinical and office operations, ensuring compliance with professional standards, agency policy, and state and federal regulatory requirements.
Key Responsibilities- Supervise and coordinate referrals, patient care, and interdisciplinary team approaches.
- Develop, implement, and update individualized plans of care, ensuring quality and compliance.
- Collaborate with clinicians and physicians on patient transfers and discharges.
- Manage KPIs, workflow, escalation/complaint calls, and guide professional clinicians on implementing Emergency Care Protocols.
- Ensure clinicians have required training, competency, and supervision, and make patient and personnel assignments.
- Participate in QA and performance improvement activities, and manage HHCAHPS distribution in SHP.
- Registered nurse with current licensure to practice nursing in the practicing state.
- Possess and maintain CPR certification.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Proven ability to work within an interdisciplinary setting.
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
- Prior use of Electronic Medical Records (EMR) preferred.
