Job Title: Occupational Therapist

About the Role:

We are seeking a skilled Occupational Therapist to join our team in providing high-quality care to patients in their home environment. As an Occupational Therapist, you will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that promote independence, safety, and well-being.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans to meet patients' unique needs.
  • Provide occupational therapy services, including exercises, activities, and education, to help patients achieve their goals.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current licensure in the State of Minnesota as an Occupational Therapist (OT).
  • Two (2) years of experience as an occupational therapist.
  • Able to travel within the geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance required.
  • CPR Certification.

Work Environment:

This position will work in the field, visiting patients in their home environment. The office is located in Roseville, MN, and the general territory for this position includes the Southwest Twin Cities Metro area.
