- Excellent communication and interpersonal skills to establish collaborative partnerships with patients, families, and caregivers.
- Ability to advocate, educate, and coach patients and families about treatment options, community resources, and psychosocial concerns.
- Facilitation of communication and coordination between healthcare delivery team members, involving patients in decision-making processes.
- Bachelor's degree in Nursing or related field.
- Certified Case Manager (CCM) or relevant certification.
- Minimum 2 years of experience in outpatient care and care management.
This role requires ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and assess patient needs.
Location and BenefitsThis position is located at Peaks Regional Office in Broomfield, Colorado.
