**Job Summary**

The Physical Therapist is a key member of our healthcare team, responsible for providing high-quality patient care services in the field of physical therapy. Under the supervision of the Director of Operations, this expert in movement disorders will be responsible for examination, evaluation, physical therapy diagnosis, prognosis, intervention, outcomes, and discharge planning for patients receiving physical therapy services.

**Key Responsibilities**

  • Provide expert physical therapy services to patients, including examination, evaluation, and treatment planning.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Conduct regular assessments to evaluate patient progress and adjust treatment plans as needed.
  • Supervise and direct personnel as needed to ensure effective patient care.

**Requirements**

  • Bachelor's Degree in Physical Therapy or related field.
  • Graduate of an accredited physical therapy professional program.
  • New Grad or entry-level experience within an ambulatory care facility or medical group practice.
  • California Physical Therapist License upon hire.
  • National Provider BLS - American Heart Association upon hire.

**Preferred Qualifications**

  • Master's Degree in Physical Therapy or related field.
  • 1 year of experience in outpatient physical therapy setting.
  • Experience with CARF and Title XXII standards.
  • EMR experience.
