Job Summary: We are seeking a skilled Nurse Practitioner or Physician Assistant to join our dynamic team in Lawrenceville, GA. In this role, you will work collaboratively with our medical director to provide top-notch patient care, engage with the care team, and drive behavior change among our geriatric patient population.
Key Responsibilities:
- Provide comprehensive primary care in an outpatient setting
- Partner with the care team to leverage coaching and integrated behavioral health models
- Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population
- Engage in weekly protected time for team building and shared discussion of challenging cases
Compensation and Benefits: We offer a competitive salary of $125,000 per year, plus a comprehensive benefits package, including medical, dental, and vision plans, as well as opportunities for professional growth and development.
