Job Summary: We are seeking an experienced Support Coordinator to join our team at NLC Support in Bergen/Passaic Counties. This is a hybrid position requiring field visits, and candidates must be flexible to travel within these counties.

About the Role: As a Support Coordinator, you will assess service needs, develop individualized service plans, and liaison with clients, families, programs, and service providers. You will also establish achievable goals, outcomes, and service eligibility, and coordinate team meetings off-site at residential and/or day programs.

Responsibilities:

  • Assessing service needs and developing individualized service plans for access to services

Requirements:

  • One year of proven experience working with adults with developmental disabilities
  • A bachelor's degree in psychology, sociology, social work, or related field
  • Candidates must be able to take the initiative, work independently, make executive decisions when necessary, and meet deadlines
  • Excellent written and verbal communication, reliable car, flexibility to attend meetings during the day
  • This position requires Covid vaccination
  • Must pass criminal background and CARI check

Benefits:

  • Competitive salary: $40,000 - $60,000 per year, commensurate with experience and caseload
