Key Responsibilities- Provide evidence-based, trimester-specific, and postpartum education to promote healthy pregnancy, delivery, and postpartum outcomes for mothers and babies.
- Create a positive experience and relationship with members and their families.
- Help members set goals and develop care plans to achieve those goals.
- Proactively engage members to manage their own health and healthcare.
- Support social determinants of health (SDOH) needs.
- Perform outreach to providers, hospitals, members, and clinics.
- Attend community events and participate in health fairs.
- Able and willing to travel within the assigned region to visit members in their homes and other settings.
- Current, unrestricted independent licensure as a Registered Nurse in Kansas.
- 2+ years of experience working within the maternal and child health population.
- 1+ years of experience with MS Office, including Word, Excel, and Outlook.
- Located in the state of Kansas.
- Ability to travel up to 75% of the time in the assigned region.
- Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
- Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection.
- Knowledge of the culture and values of community-based healthcare.
- Case Management experience.
- Background in Managed Care.
