We are seeking a highly skilled and compassionate Maternal Health Nurse Specialist to join our team at UnitedHealthcare. As a Field-Based Maternal Health Coordinator, you will play a critical role in promoting healthy pregnancy, delivery, and postpartum outcomes for mothers and babies in our Kansas communities.

Key Responsibilities
  • Provide evidence-based, trimester-specific, and postpartum education to promote healthy pregnancy, delivery, and postpartum outcomes for mothers and babies.
  • Create a positive experience and relationship with members and their families.
  • Help members set goals and develop care plans to achieve those goals.
  • Proactively engage members to manage their own health and healthcare.
  • Support social determinants of health (SDOH) needs.
  • Perform outreach to providers, hospitals, members, and clinics.
  • Attend community events and participate in health fairs.
  • Able and willing to travel within the assigned region to visit members in their homes and other settings.
Requirements
  • Current, unrestricted independent licensure as a Registered Nurse in Kansas.
  • 2+ years of experience working within the maternal and child health population.
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook.
  • Located in the state of Kansas.
  • Ability to travel up to 75% of the time in the assigned region.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
  • Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection.
Preferred Qualifications
  • Knowledge of the culture and values of community-based healthcare.
  • Case Management experience.
  • Background in Managed Care.
