- Conduct thorough patient assessments to identify physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, and cognitive needs.
- Develop and implement individualized care plans that address patient goals and promote independence.
- Educate patients, families, and caregivers on safe caregiving practices and disease management.
- Provide follow-up evaluations to ensure patient goals are met and make adjustments to care plans as needed.
