About the RoleJob Summary

We are seeking a motivated and compassionate Physical Therapist to join our team. As a key member of our interdisciplinary team, you will be responsible for assessing patient conditions, creating tailored treatment plans, and promoting healing and functional independence.

Key Responsibilities
  1. Collect and record patient care data, adhering to format and guidelines set by regulatory agencies and third-party payers
  2. Conduct initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communicate with team members
  3. Select and administer tests and measures, synthesizing evaluation results to formulate diagnoses, goals, prognosis, and plans of care
  4. Establish treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams, patients, and support systems
  5. Assess and re-assess the need for modifications to plans of care and goals, identifying consultations or referrals to other healthcare professionals or services when necessary
  6. Initiate and complete discharge plans, providing education/training to patients and/or support systems
Requirements
  • Bachelor's or Master's degree in Physical Therapy, with a Doctorate preferred
  • Licensed Physical Therapist in the state where practicing
  • Valid CPR license and experience/knowledge with EMR software
  • Excellent oral, written communication skills, clinical decision-making abilities, and a strong work ethic
