As a hospice medical social worker, you will play a vital role in providing emotional support and psychosocial evaluations to patients and their families during their end-of-life journey.

Key Responsibilities
  • Conduct thorough psychosocial evaluations to identify patient needs and develop personalized care plans
  • Provide ongoing counseling and support to patients and families, addressing their emotional, social, and spiritual needs
  • Collaborate with healthcare teams to ensure comprehensive care and coordination
  • Document patient progress and outcomes, maintaining accurate and timely records
Requirements
  • Master's degree in social work from an accredited program
  • 2+ years of clinical experience in healthcare or hospice care
  • Ability to sit, stand, bend, and move intermittently, with the capacity to lift at least 25lbs
