Job Title: RN - Home Health

We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, focusing on their physical, emotional, and social needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, communicate with the team, and maintain accurate records.
Requirements:
  • Current California Registered Nurse License.
  • 1 year of professional nursing experience, preferably in home health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Basic Life Support (BLS) certification, with the ability to obtain Advanced Cardiovascular Life Support (ACLS) certification within 6 months.
Preferred Qualifications:
  • Experience with wound care, IV therapy, and central lines.
  • Knowledge of home health care principles, including restorative care and longitudinal care.
  • Ability to work in a fast-paced environment, with a focus on prioritizing tasks and managing time effectively.
