We are seeking a skilled Nurse Practitioner to join our team in Cleveland county, NC. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes. • Conduct comprehensive assessments and develop individualized care plans for patients
• Provide routine and urgent care services, including medication management and wound care
• Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
• Stay up-to-date on relevant medical and nursing research, technology, and best practices

• 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
• Knowledge of clinical standards of care and NCQA requirements
• Ability to work independently and as part of a team
