Key Responsibilities- Provide direct patient care, including conducting health assessments, developing care plans, and implementing treatment plans.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in community health nursing.
- Current California State Board LVN License required.
- At least 3 years of experience in an ambulatory care setting or similar setting preferred.
- Graduation from a recognized practical nurse program or equivalent.
- High School Graduate (or GED equivalent).
