Position Summary:
We are seeking a qualified
Wound Care Nurse Practitioner (NP) / Physician Assistant (PA)
to provide specialized care across various skilled nursing facilities.
Key Responsibilities:
1. Manage and treat all open wounds within the facility, ensuring proper cleaning and dressing techniques.
2. Complete necessary admission documentation accurately.
3. Offer guidance and support to residents and their families regarding wound care.
4. Monitor residents' conditions and administer prescribed medications and treatments.
5. Maintain thorough documentation of resident care services in both resident and departmental records.
6. Uphold infection control policies and protocols to protect residents and staff.
7. Address resident concerns and needs through collaborative strategies with a multidisciplinary team.
8. Ensure quality of care by adhering to the facility's philosophies and standards.
9. Safeguard resident confidentiality by managing sensitive information appropriately.
10. Track infection control measures and report findings, including monthly summaries.
Qualifications:
- Valid State NP/PA License.
- Preferred experience in long-term care settings.
- Certification in wound and ostomy care is required.
- Proficient computer skills, including familiarity with MS Office.
- Strong communication abilities and a collaborative spirit.
