Position Overview:

The Speech Therapist is a licensed professional responsible for directing, supervising, assessing, and delivering therapy services to patients at home, as per the directives of the attending physician.

Required Qualifications:
  • Master's degree in Speech-Language Pathology
  • Preferred: One (1) year of relevant work experience
  • State licensure or certification as a Certified Speech Therapist in Texas
  • Pediatric and home health experience is advantageous but not mandatory
Key Responsibilities:
  • Conduct thorough evaluations and prepare evaluation reports promptly
  • Design and implement therapy activities tailored to individual needs
  • Document treatment types and patient responses in clinical/progress notes, updating the patient's record weekly
  • Maintain comprehensive records for all patients, including summary reports
  • Leverage community resources and agency personnel for appropriate referrals
  • Participate in case conferences as needed
  • Engage in staff development activities and in-service training as required
  • Confirm weekly visit schedules with the Therapy Coordinator to ensure coordination with other staff
  • Collaborate with staff, patients, and physicians in discharge planning processes
