Key Responsibilities:- Coordinate treatment and implement services for assigned clients, including outreach visits, as a member of a multi-disciplinary Early Childhood Services team.
- Consult with team members to develop individualized plans and multidisciplinary services for assigned clients.
- Maintain accurate and complete client records, including IFSPs, assessment of client functioning, and goals.
- Document client progress, required contact with collaterals and family members, and DPH forms and regulations.
- Complete billing documentation according to established schedules.
- Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions.
- Ability to work collaboratively as part of a team.
