Responsibilities

  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Monitor patient progress and adjust care plans as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
Requirements
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency.
  • Valid licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in a State, Territory, or Commonwealth of the United States, or District of Columbia.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
