About the Role

We are seeking a skilled Physical Therapist to join our Home Health team in Greeley, Colorado. As a Physical Therapist with Banner Home Health, you will have the opportunity to work with a diverse range of patients, providing personalized care and rehabilitation services in the comfort of their own homes.

Key Responsibilities
  • Evaluate patients' functional abilities and limitations, determining the need for intervention and establishing a plan of care and treatment goals.
  • Implement individualized treatment plans, considering patients' physical, social, spiritual, cultural, educational, and age-specific needs.
  • Document patient progress and treatment outcomes, ensuring accurate and timely documentation in accordance with professional practice guidelines.
  • Communicate effectively with patients, families, caregivers, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
  • Supervise and delegate tasks to physical therapy assistants, athletic trainers, and students, ensuring safe and effective delivery of care.
Requirements
  • Graduation from an APTA-accredited school of Physical Therapy or equivalent.
  • Current Physical Therapy license within the State of practice.
  • BLS certification required.
  • Ability to become proficient with computer software applications commonly used in an automated patient care environment.
