Infusion Nurse Practitioner

AleraCare is seeking a highly skilled and compassionate Infusion Nurse Practitioner to join our team in San Jose and Mountain View. As a key member of our infusion services team, you will provide high-quality patient care and work collaboratively with our healthcare professionals to deliver exceptional patient outcomes.
Responsibilities:- Provide comprehensive nursing services to patients receiving infusion therapy, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized plans of care that address the unique needs and goals of each patient.
- Collaborate with our multidisciplinary team to ensure seamless care coordination and communication.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Administer medications and treatments as prescribed by physicians, and monitor patient responses to ensure safe and effective care.
- Provide education and support to patients and their families to promote self-care and independence.
- Participate in performance improvement initiatives to enhance patient care and outcomes.
- Stay up-to-date with the latest clinical guidelines and best practices in infusion therapy.
- Graduate of an accredited school of nursing and current NP state license.
- At least one year of nursing experience, with a preference for infusion nursing experience.
- Effective interpersonal, time management, and organizational skills.
- Strong communication skills in speaking and writing.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
