**Job Summary:**

We are seeking a highly skilled Housing Transition Specialist to join our team at the Economic Opportunity Council of Suffolk, Inc. This individual will be responsible for providing transitional housing assistance to clients, enabling them to gain access to medical care and treatment.

About the Role:

  • The Housing Transition Specialist will provide short-term or emergency housing assistance to clients, developing an individualized housing plan to guide their linkage to permanent housing.
  • This role requires excellent communication skills, both on the phone and in face-to-face situations, as well as the ability to maintain strict consumer confidentiality.
  • The ideal candidate will have experience in case management, with knowledge of community resources and referrals for housing programs.
  • This position also requires strong organizational skills, with the ability to prioritize tasks and meet deadlines.

Responsibilities:

  • Respond to incoming calls from potential clients, conducting initial screenings/intake to determine eligibility for program services.
  • Complete all necessary intake paperwork, collect client identifying documentation, and provide an initial program orientation.
  • Refer clients in need of transportation to the Transportation Coordinator, collaborating with them to provide support services.
  • Conduct needs assessments with clients, identifying urgent needs/social determinants of health.
  • Assist clients in determining eligibility and applying for emergency financial assistance programs and/or make appropriate referrals to outside resources/utilize the Unite Us referral platform.
  • Develop individualized housing plans, including financial planning and housing goals collaboratively with each client.
  • Facilitate and conduct housing searches with clients, assisting with application completion when necessary.
  • Provide case management to clients, supporting forward progress on goals aligned with their individualized housing plan.
  • Monitor clients' progress towards goals, tracking activities and ensuring timely and accurate collection of program data.
  • Collect and input client data into agency-approved databases in a timely manner.

Requirements:

  • Associates degree in Human Services or related field and/or 5 years' experience accepted in lieu of degree.
  • Strong written, oral communication, and public speaking skills.
  • Dedicated to details and masterful at multi-tasking, with the ability to organize and prioritize tasks to meet deadlines.
  • Knowledge of community resources and referrals for housing programs, entitlement programs, and additional client-centered referrals.
  • Bilingual (English/Spanish) a plus.
  • Ability to work effectively with people of diverse backgrounds and cultures.
  • Familiar with Word, Excel, and PowerPoint and experience with data management systems.

Salary: $45,000 - $60,000 per year

Educational Requirements:

  • Associates degree in Human Services or related field and/or 5 years' experience accepted in lieu of degree.
