Key Responsibilities- Assist patients with daily living activities, including bathing, dressing, and grooming, while promoting independence and dignity.
- Provide nursing care to patients, including vital sign monitoring, wound care, and medication administration, under the supervision of a Registered Nurse.
- Document patient care accurately and thoroughly, including progress notes and medication administration records.
- Collaborate with healthcare team members to ensure seamless patient care, including communicating with physicians, nurses, and other healthcare professionals.
- Perform and document patient rounds, including vital sign monitoring and medication administration, to ensure accurate and timely documentation.
- Report changes in patient condition to the Registered Nurse, including any concerns or issues that may impact patient care.
- Assist with patient transfers to other institutions or facilities, including preparing necessary documentation and equipment.
- Maintain a clean and safe environment by cleaning and checking equipment, and reporting any maintenance or repair issues to management.
- CNA Certificate or CNA State License required.
- BLS certification required.
