Compassionate Home Care Nurse

We are seeking a skilled Registered Nurse to join our Home Care team at AgeSpan. As a key member of our interdisciplinary team, you will be responsible for providing exceptional care and support to our clients in their homes.
Job DescriptionThe Registered Nurse: Home Care is responsible for participating in the case management of elders living in the community. This includes assessing consumers for personal care services, determining clinical eligibility for community programs, and developing community care plans in accordance with guidelines set by the Executive Office of Elder Affairs, MassHealth, and agency standards.
Key Responsibilities:- Perform on-site evaluations of consumers in their homes and nursing facilities, including review of medications and clinical assessment.
- Conduct field visits to assess an older adult's physical, social, emotional, and environmental status to determine their eligibility and needs.
- Participate in interdisciplinary team meetings and attend case consultations and conferences.
- Determine clinical eligibility for MassHealth recipients or applicants for nursing home placement, adult day health, home and community-based waiver services, and other community-based programs.
To be successful in this role, you will need a valid Massachusetts Registered Nurse license, a Bachelor of Science in Nursing or Associate of Science in Nursing degree, and at least one year of clinical experience and/or nursing in a community or long-term care setting.
