Care Coordinator for Quality Senior Living

Job OverviewAs a Resident Assessment Coordinator/MDS Manager, you will be responsible for coordinating the Resident Assessment Instrument (RAI) process, completing Material Data Sets (MDS), and maintaining accurate records. You will also provide training and education to the skilled nursing clinical team members and facilitate In Care Plan / Rehabilitation Conferences.
Responsibilities:- Ensure accurate completion of the RAI using information in the health information record, ADLs, Flow Sheets, 24 Hour Report, and others as appropriate.
- Complete MDS in accordance with Federal, State, and Local policies, regulations, and guidelines.
- Maintain schedule of MDS, RAPs, Care Plans, and assessments.
- Coordinate and facilitate In Care Plan / Rehabilitation Conferences.
- Provide training and education to the skilled nursing clinical team members regarding MDS / Care Plan process and required documentation.
- Graduate of approved college/school of nursing.
- Maintains a current state license as a Professional Registered Nurse (RN), or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- One year supervisory experience in long-term care environment which may include recruiting staff, coaching, performance management, responsibility of daily department operations.
- Material Data Set (MDS) and Care Plan Development experience preferred.
We offer a competitive compensation package including a salary range of $65,000 - $85,000 per year, depending on location, skills, experience, qualifications, and certifications/licenses (where applicable).
