About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Navigator. This is a per diem/variable position that offers flexibility and variety in your work schedule.

Job Summary

The Patient Care Navigator will provide high-quality patient care and navigation services to patients and their families. This role will require collaboration with healthcare professionals, hospitals, and community resources to ensure seamless care coordination.

Responsibilities
  • Assess patients' needs and develop personalized care plans
  • Collaborate with healthcare teams to ensure effective communication and coordination
  • Navigate patients through the healthcare system, addressing any barriers or challenges they may face
  • Provide education and support to patients and families on disease management and self-care
  • Maintain accurate records and reports of patient interactions and outcomes
Requirements

To be successful in this role, you will need:

  • Bachelor's Degree in Nursing (BSN)
  • Current California Registered Nurse License
  • Minimum 2 years of acute hospital experience, including 1 year of supervisory experience
  • Strong knowledge base or experience in specialty area assigned
