Key Responsibilities- Plan and administer prescribed physical therapy treatment and training for patients to attain the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment and the overall goals of the patient/rehab team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
- Provide Physical Therapy Assistants and Home Health Aides with written instructions and care plans that reflect current plans of care.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Master's Degree in Physical Therapy (or grandfathered licensure prior to masters requirement).
- Current license to practice physical therapy and maintains licensure requirements, including 24 hours of continuing education.
- Complies and maintains current CPR certification.
- One year of appropriate experience as a physical therapist.
- Excellent verbal and written communication skills.
- Sensitivity to cultural diversity and making accommodations available to provide services to a diverse population.
