At MedCentric, we are dedicated to enhancing the quality of life for older adults with complex health needs. We are looking for a committed Registered Nurse to deliver exceptional, holistic care as part of our PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team.
In this pivotal role, you will manage and provide direct nursing services to PACE participants across various environments, including clinics, care centers, private residences, virtual consultations, skilled nursing facilities, and beyond. As an essential member of the interdisciplinary team, you will work collaboratively with other healthcare professionals to conduct thorough assessments, create tailored care plans, and ensure the timely provision of services.
This position offers the chance to practice at the highest level of your licensure, fostering continuous learning and the advancement of your professional skills. You will have the opportunity to personalize the care experience, focusing on the unique needs and preferences of each participant.
Key Responsibilities:
- Act as the primary contact for the interdisciplinary team, ensuring progress on participants' care plans
- Administer nursing care in diverse settings, including primary care, medication management, wound care, oxygen therapy, and more
- Perform comprehensive nursing evaluations of physical, psychosocial, and behavioral health
- Recognize and independently initiate emergency interventions as necessary
- Educate participants, caregivers, and staff on individualized care plans and response strategies
- Monitor and assess therapeutic interventions, recommending modifications to enhance health and well-being
- Provide on-call support as required
Ideal Candidate:
- Strong commitment to serving high-risk seniors and frail older adults
- At least 2 years of experience in caring for adults, chronically disabled or elderly populations
- Ability to operate independently in managing clinical needs and program functions
- Excellent active listening and conflict resolution skills
- Flexibility to adapt to various clinical environments
- Training in wound care, IV therapy, and blood draws
- Active, unrestricted RN license in California
- BSN preferred
- CPR and First Aid certification
- Alzheimer's certification
- Proficiency in English and Spanish is preferred
