**Key Responsibilities:**

  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Administer medications, treatments, and therapies as prescribed by physicians, ensuring accurate and timely delivery.
  • Provide education and training to patients and their families on self-care techniques, disease management, and healthy lifestyle habits.
  • Collaborate with healthcare teams to coordinate care, share information, and ensure seamless transitions.

**Requirements:**

  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Florida.
  • CPR certification and OSHA certification.
  • Valid in-state driver's license and current registration and insurance.
  • Driving record with no more than 2 points within the last 3 years.
