About the Role

We're seeking board-certified Nurse Practitioners to conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members. As a valued member of our team, you'll have the opportunity to work independently, conducting thorough assessments and developing personalized care plans that address the unique needs of each patient.

  • Conduct in-home visits to assess patients' overall health and well-being

Requirements
  • Master's degree in Nursing (Required)
  • Current Pennsylvania Nurse Practitioner License (Preferred)
