We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Case Manager at Lakeview Hospital. As a Case Manager, you will play a vital role in coordinating patient care, ensuring seamless transitions, and promoting quality outcomes.
Key Responsibilities- Coordinate patient care plans, managing length of stay and resource utilization.
- Collaborate with multidisciplinary teams to develop safe and effective discharge plans.
- Facilitate patient progression and transition of care using established criteria.
- Identify and address the needs of under-resourced patient populations, including patient education and community-based resources.
- Partner with Social Services to ensure post-acute medical needs and levels of care are appropriate for patients at risk for adverse outcomes.
- Current licensure as a Registered Nurse in the State of Utah or RN compact license from participating compact states.
- Associate Degree in Nursing required; BSN preferred.
- 2+ years of experience in case management.
- American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification.
- Certification in case management or utilization review preferred.
- InterQual experience preferred.
