Clinical Care Specialist

The Department of Clinical Services at Community Care Physicians is seeking a skilled clinical professional to join our team. As a key member of our centralized clinical team, you will provide high-quality clinical support to our organization, adapting to the evolving needs of our clinical practices.
Key responsibilities will include providing direct clinical care to patients in-office or via telemedicine, covering leaves of absence as needed. You will also support our task management program, annual wellness visit program, and other clinical services as necessary. Compensation and Benefits
CCP offers a competitive salary range of $100,000-$150,000, with opportunities for professional and financial growth. Requirements
To be considered for this role, you must be certified and actively registered as a Nurse Practitioner or Physician Assistant by the New York State Education Department (NYSED). A minimum of one year of experience in Primary Care is preferred, and current CPR certification is required.
