Job SummaryWe are seeking a Clinical Social Worker to join our Senior Care Network team. The successful candidate will provide case finding, consultation, and care management services to adults with disabilities and older adults and their families in home and community-based settings.
Responsibilities- Assess needs, monitor quality of care, and provide education, advocacy, and support to clients and their families.
- Work with a multidisciplinary team to provide comprehensive care.
- Develop and implement care plans to preserve health, maintain function, and promote independence.
- Collaborate with community resources and public benefits to ensure access to necessary services.
- Master's degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or a related field.
- Minimum 2 years of experience working with older adults and/or adults with disabilities.
- Strong clinical assessment skills and knowledge of community resources.
- Ability to work independently and as part of a team.
- Competitive compensation package.

Compensation Range: $84,000 - $100,055 per year, depending on qualifications and experience.
