About the Position:

The Enhanced Care Management (ECM) Community Health Worker is a critical member of our care team, responsible for coordinating medical, behavioral, and social services to improve health outcomes and enhance patient satisfaction. This role requires a unique blend of clinical knowledge, cultural competence, and community engagement skills to effectively support patients with complex and chronic conditions.

The ECM CHW will work closely with our multidisciplinary team to develop and implement personalized care plans, address barriers to care, and promote collaboration between patients and their healthcare providers. Key Responsibilities:

  • Coordinate care services and develop personalized care plans for patients with complex and chronic conditions
  • Address barriers to care and promote collaboration between patients and their healthcare providers
  • Engage with patients in a culturally sensitive and compassionate manner, utilizing evidence-based approaches to promote positive change
  • Collaborate with our multidisciplinary team to develop and implement innovative solutions to address the complex challenges faced by our patients
  • Monitor treatment adherence and provide support to patients in navigating the healthcare system and accessing community resources

Requirements:

  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations
  • Knowledge of contract parameters, objectives, milestones, and other deliverables
  • Ability to integrate budgetary and service utilization data, forecast expenditures, and respond to ensure both effective provider utilization and budget compliance
  • Ability to work constructively with diverse people and with parties that may have divergent perspectives and interests
  • Strong oral and written communication skills, with the ability to prepare complex reports, policies, and procedures
