Job Summary

The Care Support Specialist plays a vital role in ensuring the smooth operation of patient throughput. Reporting to the Nursing & Clinical Director, this individual will be responsible for maintaining ongoing communication with various departments to contribute to an efficient process.

  • Coordinate continuous communication with nursing units, environmental services, and patient flow coordinator/nursing supervisors.

  • Utilize the electronic bed-board to move patients into assigned beds effectively.

  • Provide timely patient information regarding transfers/admissions to nursing units.

  • Notify Environmental Services promptly when a bed is ready to be cleaned, and expedite cleaning as needed.

  • Inform Patient Flow Coordinator/Nursing Supervisor about bed readiness status immediately.

  • Notify relevant staff (chain of command) when issues arise related to patient placement and bed cleaning.

  • Demonstrate effective resource management utilizing behaviors inherent to the Care Support Specialist role.

  • Establish office workflow and communication protocols.

  • Answer phone calls accurately, take messages, and provide them to the right personnel in a timely manner.

  • Assist with staffing needs by making phone calls to secure additional staff or call off staff as directed by the Patient Flow Coordinator/Nursing Supervisor.

  • Help with filing updates in nursing manuals as necessary.

  • Enhance efficiency by improving automated data collection processes.

  • Contribute to PI data entry and collection for capacity throughput initiatives.

  • Schedule appointments and coordinate referrals between facilities.

  • Ensure accurate documentation and communication of transfer status.

  • Collaborate with clinical teams to facilitate patient care and support.

  • Maintain relationships with healthcare providers and stakeholders.
