Job Summary:The ideal candidate will have a strong background in geriatric care, population health management, and home-based healthcare. You will be responsible for conducting thorough assessments, developing and implementing treatment plans, and providing education and support to our members and their families.
Responsibilities:- Conduct in-home assessments and develop comprehensive treatment plans for members with acute and chronic conditions
- Provide education and support to members and their families on disease management, medication adherence, and healthy lifestyle choices
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and communication
- Stay up-to-date with the latest clinical guidelines, research, and best practices in geriatric care and population health management
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes
- Current and valid State of North Dakota RN license required
- Bachelor's degree in Nursing and North Dakota Advanced Practice Nursing license required; Master's degree in Nursing preferred
- Board Certification from a nationally recognized certifying body for Nurse Practitioners required
- Unrestricted prescribing and dispensing privileges with DEA licensure required
- 2 years' experience as Advanced Practice Nurse preferred
- Experience working with adult, older adult, geriatric population preferred
- Experience and understanding of population health management preferred
