Job Overview

We are seeking a skilled Nurse Case Manager to join our team at Northwell Health. This role will involve coordinating the delivery of routine and specialty health care services to address the assessed needs of individuals receiving services.


Key Responsibilities:
  1. Coordinate Health Care Services: Ensure the effective delivery of health care services, maintaining accurate health care profiles and updating them as necessary.
  2. Health Care Assessments: Conduct semiannual assessments or as indicated to establish health care outcomes for individuals receiving services.
  3. Liaison Role: Communicate health information between team members, families, other agencies, hospitals, clinics, and service providers.
  4. Plan of Nursing Service (PONS): Develop individualized plans based on health care needs, update as required, and ensure staff training.
  5. Documentation and Record-Keeping: Maintain up-to-date health care progress documentation and electronic health records in compliance with regulations and policies.
  6. Training and Development: Conduct general and individual-specific health-related training for staff and other team members as required.
  7. Medication Management: Assign Direct Care staff to perform health care treatments/procedures, initially train and regularly monitor their performance.
  8. Nursing Coverage: Provide on-site, electronic, or cell phone coverage between 7:00 am - 7:00 pm, Monday through Friday.
  9. Handover and Compliance: Ensure complete and up-to-date handovers to On-Call nursing service, including emerging and acute situations, and maintain compliance with medical standards.
  10. Meetings and Committees: Participate in meetings and committees as assigned, including IDT, Day Program, Human Rights, Departmental, and specially arranged meetings/committees.
