Job SummaryWe are seeking a highly skilled Behavioral Health Registered Nurse RN Home Health Specialist to join our team. As a key member of our care team, you will be responsible for providing exceptional patient care and support to individuals with mental health needs in the comfort of their own homes.
Key Responsibilities- Assess patient needs and develop personalized care plans to meet their unique requirements
- Provide skilled nursing care and support to patients with mental health conditions, including medication management and therapy
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
- Conduct regular patient assessments and evaluations to monitor progress and adjust care plans as needed
- Document patient information and care activities in a timely and accurate manner
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One year of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service
- Valid CPR certification and a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
- Ability to travel to all business locations
