Job Summary

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team as a Private Duty Nurse. As an LVN with At Home Healthcare, you will work closely with our RN Case Managers to deliver high-quality patient care and support in the home setting.

Responsibilities

  • Collaborate with the RN Case Manager to develop and implement patient-specific care plans.
  • Deliver skilled care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Document patient care delivery in our electronic medical record (EMR) system.
  • Contribute to a culture of caring through individual accountability and teamwork.
