Job Overview

We are seeking a skilled and compassionate Registered Nurse to join our home health care team in Capitol Hill. As a Visiting Hospice Nurse, you will provide high-quality patient care and services in the comfort of patients' homes.

About the Role

This is a 40-hour per week position that requires strong communication skills, attention to detail, and the ability to work independently as part of an interdisciplinary team. You will be responsible for assessing patient needs, developing personalized care plans, and providing education and consultation on pain and symptom management.

Key Responsibilities
  • Collaborate with patients, families, and healthcare professionals to plan and implement effective care strategies.
  • Provide skilled nursing interventions, including injections, dressings, catheter insertion and care, colostomy and tracheotomy, central line IV care, and blood draws.
  • Conduct thorough assessments, develop and implement individualized care plans, and educate patients and caregivers on disease management and adjustment to illness.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.
Requirements
  • Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings.
  • Associates degree in Nursing from an accredited program.
  • Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire.
  • Driver's License (Washington) required at hire.
  • Basic Life Support certification required at hire.
