About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hastings Senior Health & Living. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Responsibilities
  • Assess, plan, implement, and evaluate care plans for residents, recognizing and managing common geriatric syndromes.
  • Supervise the work performed by TMAs and Nursing Aides, ensuring that care is delivered with compassion and respect.
  • Administer medications and/or treatments in accordance with physician's orders, monitoring for effectiveness and response.
  • Manage medications, supplies, and equipment, including ordering, receiving, storing, and disposing of items in accordance with policies and procedures.
  • Document care using computerized medication administration and charting software, ensuring accurate and timely documentation.
  • Notify Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, taking follow-up action as necessary.
Requirements
  • Current MN LPN license and good standing.
  • Excellent oral and written communication skills, with the ability to prioritize tasks and focus on quality resident care.
  • Skills in problem definition and resolution, with a commitment to delivering compassionate care.
