Glens Falls Center is a 120-bed rehabilitation and skilled nursing facility located in Glens Falls, NY. Our mission is to provide high-quality rehabilitation and skilled nursing services, with a focus on excellent patient outcomes and a warm, welcoming environment.

Responsibilities:

  • Conduct comprehensive admission assessments, including medical, social, and psychological evaluations.
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
  • Coordinate the admission process, including scheduling appointments, reviewing records, and communicating with family members and healthcare providers.
  • Assist with the development and direction of the Nursing Department, as directed by the DON.
  • Review and evaluate resident care plans, making recommendations for changes as necessary.
  • Perform required charting and maintain accurate, up-to-date records.
  • Participate in discharge planning and coordinate with healthcare providers to ensure smooth transitions.

Requirements:

  • Bachelor's degree in Nursing or related field.
  • Active, unrestricted NYS Registered Nurse License.
  • Minimum 2 years of experience in long-term care, with a focus on admissions and coordination.
  • Excellent clinical assessment skills, with ability to make sound judgments.
  • Strong leadership and supervisory skills, with ability to motivate and direct staff.
  • Proficient in MS Office (Excel, Word, Outlook) and electronic medical records systems.
  • Ability to work independently, with strong organizational and time management skills.

Estimated Salary: $65,000 - $80,000 per year, depending on experience.
