Salary Range
The salary for this role is $26.38-$38.26 per hour, reflecting the expertise and qualifications required to excel in this position.

About the Role
We are seeking a skilled Registered Nurse (RN) to join our team as a Healthcare Specialist - Annual Wellness Visits. In this role, you will work collaboratively with healthcare providers to deliver high-quality patient care, focusing on preventive health maintenance and chronic disease management. Your primary responsibility will be to conduct annual wellness visits for Medicare patients, ensuring they receive comprehensive health assessments and personalized preventative care plans.

Essential Responsibilities
This role involves working closely with patients to gather health and family history, vital signs, health screenings, and yearly goals. You will also collaborate with healthcare providers to create customized preventative screening plans for Medicare patients. Additionally, you will provide education to patients and families on health status, the importance of health maintenance, and management of acute and chronic conditions. Other key responsibilities include facilitating conversations on advance care planning, providing patient care based on practice guidelines, and participating in multidisciplinary teams to improve patient care processes and outcomes.

Qualifications
To succeed in this role, you must have a current Registered Nurse (RN) license that allows you to practice in our state, a graduate degree from an accredited nursing program, and Basic Life Support (BLS) certification from the American Heart Association. Your ability to communicate effectively with patients and healthcare teams, combined with your commitment to delivering high-quality patient care, makes you an ideal candidate for this opportunity.
