We are seeking an experienced RN Case Manager to join our team at All Medical Personnel. This is a rewarding opportunity for a skilled professional to make a meaningful difference in the lives of patients and their families.
Job SummaryThe successful candidate will be responsible for coordinating medical care, ensuring seamless transitions between healthcare settings, and promoting cost-effective outcomes. As a key member of our interdisciplinary team, you will work collaboratively with physicians, nurses, and other healthcare professionals to deliver high-quality patient care.
Key Responsibilities:- Coordinate patient care across the continuum, from hospital admission to discharge and beyond.
- Develop and implement individualized care plans that address patient needs, preferences, and cultural requirements.
- Collaborate with healthcare teams to ensure timely and efficient care delivery, minimizing delays and adverse events.
- Educate physicians and other healthcare providers on InterQual criteria, patient acuity, and treatment plans.
- Ensure compliance with regulatory requirements, including CMS, JCAHO, and NYS DOH standards.
To be successful in this role, you will need:
- 5-6 years of experience in case management or a related field, preferably in an acute care hospital setting.
- Expertise in InterQual and Millman, with a strong understanding of discharge planning principles.
- Familiarity with all patient populations, including those with complex needs.
- Certification in case management or utilization review is highly desirable.
- Proficiency in electronic medical records systems, such as MediTech.
