Job Summary

We are seeking a skilled Hospice RN On Call to provide exceptional care and support to our patients and their families in the comfort of their own homes and various nursing facilities.

This is a full-time, part-time per diem position with a flexible schedule, requiring the ability to work independently and as part of a team.

Responsibilities and Duties
  • Provide on-call support after agency hours for hospice patients and families, addressing comfort, symptom management, medication management, and DME needs.
  • Coordinate care between the hospice interdisciplinary team and facilities, ensuring seamless transitions and effective communication.
  • Collaborate with the triage nurse to determine the need for on-site visits and provide direct care and support as required.
Qualifications and Skills
  • Current licensure by the Ohio State Board of Nursing and a graduate of an accredited school of nursing.
  • Excellent time management skills, ability to multitask, and a self-starter mentality.
  • CPR certification and a valid driver's license with current insurance coverage.
Schedule and Requirements
  • Night shift and on-call responsibilities.
  • Weekends and flexible scheduling.
