Discharge Planner LPN Job Description

Bethel Health & Rehabilitation Center is seeking a skilled Discharge Planner LPN to join our team. As a Discharge Planner LPN, you will play a critical role in coordinating and facilitating the safe and timely discharge of patients from our nursing facility.

Key Responsibilities:

  • Collaborate with healthcare professionals, patients, and their families to develop comprehensive discharge plans that meet the physical, emotional, and social needs of patients post-discharge.
  • Assess financial resources available for patients and make referrals to appropriate community resources.
  • Provide education and support to patients and their families to ensure a smooth transition to post-discharge care.
  • Work collaboratively with the interdisciplinary team to ensure that patients receive the necessary care and services to meet their needs.

Requirements:

  • Minimum of LPN Nursing License or Degree in Social Work, Experience in Healthcare Administration, or a related field.
  • Relevant experience in discharge planning, case management, or a related healthcare field.
  • Knowledge of healthcare regulations, including Medicare/Medicaid guidelines and insurance coverage requirements.
  • Familiarity with community resources and support services available for post-discharge care.
