Nurse Navigator RN Job Summary

This role involves linking mobile resources together to respond appropriately to patient needs. The Nurse Navigator RN will be responsible for telephonic patient triage, navigating care across the healthcare continuum, and coordinating team care in a virtual setting.

Key Responsibilities:
  • Telephonically triage patients and navigate care based on evidence-based protocols.
  • Patient education across the healthcare continuum.
  • Telemonitoring activities engaging healthy patient behaviors.
  • Effective communication and collaboration with team members, patients, caregivers, referral sources, physicians, and facility staff.
  • Ability to learn and use computer and required software programs.
  • Participate in activities promoting the AMR Integrated Healthcare Program and the AMR organization.
Requirements:
  • Compact RN License.
  • 5 years or more of RN experience in Emergency or Acute Care setting.
  • Experience with clinical data gathering and Ethical Principles.
