We are seeking a highly skilled Social Worker to join our team at the Veterans Affairs, Veterans Health Administration. As a Social Worker, you will play a critical role in providing essential services to homeless Veterans and their families.
Key Responsibilities- Identify and engage homeless Veterans, providing them with a range of social work services, including assessment and referrals, clinical treatment, short-term case management, advocacy, and service coordination.
- Work closely with interdisciplinary teams to develop and implement comprehensive treatment plans, addressing the unique needs of each Veteran.
- Provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
- Conduct psychosocial assessments with Veterans from diverse backgrounds, identifying strengths, limitations, coping skills, and internal/external supports and service needs.
- Develop and implement treatment plans, identifying problems, strengths, weaknesses, coping skills, and assistance needed.
- Make appropriate referrals to community and other agencies, coordinating services as needed, and acting as an advocate with VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregivers.
- Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Licensure or certification in a state to independently practice social work at the master's degree level.
- Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds.
- Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
- Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
- Experience in drug and alcohol treatment.
This position is based at the San Francisco VA Health Care System and requires regular interaction with Veterans, family members, and healthcare staff. The incumbent must be able to work in a fast-paced environment and maintain confidentiality and professionalism at all times.
The selected candidate will be required to complete an online onboarding process and pass a pre-employment physical examination. Participation in the seasonal influenza vaccination program and the Coronavirus Disease 2019 (COVID-19) vaccination program is also required.
