Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team of compassionate caregivers. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to patients in the comfort of their own homes.

Responsibilities
  • Deliver high-quality, patient-centered care to patients with chronic and acute illnesses or injuries
  • Conduct initial patient screenings, physical examinations, and create assessment plans
  • Address urgent care needs and collaborate with community teams to complete care assessments
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and ensure accuracy at each patient appointment
  • Deliver palliative care and end-of-life care to patients and families
  • Facilitate family meetings and provide counseling and guidance for patients with critical conditions
Requirements
  • Master's Degree in Nursing
  • Valid nurse practitioner license in the state of operations and current board certification in your field
  • 5+ years of progressive care experience
  • BLS and ACLS certification and an NPI# as an NP
  • Valid driver's license, car insurance, and willingness to drive to patient homes/patient locations
